Enrolment guide

How to fill in the enrolment form

Our online enrolment form is in English. This guide takes you through it step by step, so you can keep it open alongside the form.

The form opens in a new tab, so this guide stays open here.

Before you start

  • About 15–20 minutes. From step 3 you can press “Save and submit later” and come back to it.
  • A clear photo or scan of your ID (JPG, PNG or PDF, up to 5MB). If you are not a New Zealand citizen, you will usually need your passport and your visa.
  • Your passport’s expiry date.
  • Your NHI number, if you know it (it is fine if you don’t).
  • The name of the pharmacy you use.
  • Your mobile number and email address.
  • The name and mobile number of your next of kin.
  • If you work: your employer’s name and workplace address.

Three things to know

  1. Type everything in English letters. Write your name exactly as it is on your passport.
  2. Addresses are found by typing the street number and name, then choosing from the list that appears.
  3. Each person needs their own form, including children. For a child, fill in the child’s details; the parent signs at step 10.

Want to read the form in your language?

Your browser can translate the page for you: in Chrome, use “Translate” in the menu; on an iPhone, tap the “aA” button in Safari’s address bar. Use the translation only to understand the questions — answer in English, and treat the English wording as the one that counts.

Step by step

  1. Step 1 · Welcome to Our Practice

    Choose how to register

Choose to enrol, then open and accept the privacy information.

    On the formEnrolled Patient
Make Newa your regular GP. Choose this one to enrol.
    On the formCasual Patient
    Visit occasionally without enrolling.
    On the formterms and conditions
Opens the national statement on how your health information is used: why it is collected, who it is shared with, and your right to see and correct it. Read it and press “Accept” at the bottom.

    Watch out

    • The “Next” button stays grey until you have pressed “Accept”.
  2. Step 2 · Eligibility

    Confirm you can enrol

Enrolment is for people eligible for publicly funded health care in New Zealand.

On the formI am entitled to enrol because I am residing permanently in New Zealand.
Tick this if you plan to live in New Zealand for at least 183 days in the next 12 months.
On the formAre you a New Zealand citizen?
    Yes or No.
On the formI hold a resident visa or a permanent resident visa
If you answered No: tick this if you hold a resident or permanent resident visa.
On the formI have a work visa/permit and can show that I am able to be in New Zealand for at least 2 years
For work visa holders who can be in New Zealand for at least two years (earlier visas count).
On the formI am under 18 years and in the care and control of a parent/legal guardian…
For children whose parent or guardian meets one of the other criteria.
On the formI understand I will need to provide documentation to prove my eligibility status
Tick this. You will upload your documents at step 9.

    Watch out

    • Other options cover Australian citizens and residents, interim visas, refugees and protected persons, and some scholarship holders. If none fits, call reception before you continue.
    • Your answers here decide which documents step 9 asks for.
  3. Step 3 · Personal Info

    Your personal details

Boxes marked * must be filled in.

    On the formNHI (if known)
Your National Health Index number: 3 letters and 4 more characters, e.g. ABC1234. Leave it blank if you don’t know it.
    On the formFirst/Given Name · Last/Family Name
In English letters, as on your passport. Your family name goes in “Last/Family Name”.
    On the formDate of Birth
    Day, month, year — e.g. 05 / 09 / 1985.
    On the formCountry of Birth
Type, then choose from the list. Mainland China: “China (People’s Republic)”. Taiwan: “Taiwan”. Hong Kong: “Hong Kong”. South Korea: “Korea, Republic of (South Korea)”.

    Watch out

    How to reach you

    Your phone, email and address.

    On the formCell Phone

On the formPreferred Contact Methods · Select Preferred Contact
Some methods are already ticked; change them if you like. Then choose one as your main way to be contacted.

    Watch out
